-My car starts fine, but after about 5 minutes of running normally the temperature gage goes from the bottom to halfway, stays there for about a minute or two, then slowly rises to above the heat line and the car says it is overheating.
This problem started after I changed a top radiator hose, but I'm positive I did it properly and it isn't leaking fluid anywhere.
The car's oil and coolant levels are both normal so that isn't the cause of this problem.
There are no other symptoms, the car isn't ejecting any smoke from the exhaust or anything like that..

My windstar requires that the thermostat "jiggle valve" be positioned properly.

...you may be able to bleed air, at the coolant temp sensor.
Your problem may be as simple as having some air in your cooling system.
